What a muscle knot actually is: the physiology of stress
A muscle knot is a patch of muscle that stays contracted after the tension is no longer needed. A healthy muscle works in cycles: it tightens, does its job, and lets go. Chronic stress breaks that cycle. The "release" signal from your nervous system comes less and less often, and the muscle holds a low-grade contraction for hours, then days. After a while, you simply stop noticing it.
Then the mechanics kick in. A muscle that's always contracted squeezes its own capillaries and gets less blood flow. Metabolic waste builds up in the tissue, and tender, ropy spots form: those are trigger points. That's where the familiar lineup comes from: an aching neck by evening, tension headaches, a back that feels like a plank in the morning.
The idea that "emotions are stored in your muscles" comes from body-oriented therapy: back in the 1930s, Wilhelm Reich described "body armor," feelings supposedly frozen into the body. Science is skeptical of that wording: an emotion is a process in your nervous system, and it doesn't have a storage unit in your trapezius. But there's a real, measurable mechanism behind the metaphor: your brain controls muscle tone, and an anxious brain keeps the body on low-level alert for years. That's plain stress physiology, no mysticism required. Pop psychology calls this "emotional blocks." The label is loose, the mechanism is the same.
Where tension builds up most, and why
Your jaw. Pound for pound, the jaw muscles are among the strongest in your body. Clenching your teeth is an ancient response to threat and effort: we do it when we're angry, gritting through something, or concentrating hard. By day it's a locked jaw at your desk; by night it's bruxism (teeth grinding in your sleep). Telltale signs: pain in your temples or near your ear, a clicking jaw joint, teeth that feel sensitive in the morning.
Your neck and shoulders. When you're startled, you instinctively pull your head down into your shoulders to protect your neck. When stress runs in the background all day, that protective posture becomes permanent: shoulders slightly raised, traps holding tension from morning to night. Add hours at a screen with your head craned forward, and you get the most common tension zone for desk workers.
Your diaphragm. The "freeze and hide" response literally stops your breath: when you're scared, you hold the inhale and shift to shallow chest breathing. A chronically tight diaphragm creates that stuck feeling under your ribs, makes a full deep breath feel impossible, and feeds the sense of tightness in your chest.
Your lower back. Two factors meet here: hours of sitting, plus the stress "crouch": under pressure, people slump a little and tuck the pelvis. Your lower-back muscles stabilize that setup for years without a break, which is why long-running stress often leaves your lower back aching even with zero physical exertion.

Anxiety is a state of readiness for a threat. Your brain doesn't care that the threat is abstract: a deadline, a conflict, money uncertainty. It preps your body the same way it did thousands of years ago: it tenses your muscles to strike or to run. The problem with modern stress: no strike and no run ever happen. The tension gets mobilized, never spent. It just stays in your body.
Then the loop closes. Your brain constantly reads signals from your body, and tense muscles register as confirmation that danger is nearby: if the body's braced for combat, there must be something to brace for. Anxiety ticks up, the muscles clamp down harder, and the "danger" signal gets louder. Stress turns into tension, tension into pain, pain into fresh stress.
There's a third layer: habituation. After a few weeks, background tension stops registering at all, like the hum of a refrigerator you only notice once it shuts off. You genuinely believe you're relaxed, right up until your head or neck starts to hurt. So step one is learning to notice tension again; step two is releasing it regularly.

Progressive muscle relaxation, step by step
American physician Edmund Jacobson developed the method in the 1920s, and it's one of the best-researched relaxation techniques, used for anxiety disorders, insomnia, and tension headaches. The principle sounds backwards at first: to relax a muscle, you tense it hard on purpose. After a strong contraction, the muscle releases deeper than its starting point, and the contrast teaches you to tell "clenched" from "let go." That's the skill chronic tension takes away.
The basic formula: tense one muscle group for 5 to 7 seconds (hard, but never to the point of pain), then release all at once and spend 20 to 30 seconds just noticing the sensation. Tense on an inhale or a breath hold; release with the exhale. A full pass through the body takes 12 to 15 minutes.
- Set up. Find 15 minutes when nobody needs you. Sit in a chair that supports your head, or lie down. Phone away, belt and tight clothes loosened. Follow the list at first. The sequence sticks quickly.
- Hands and forearms. Squeeze both fists as hard as you can and feel the tension run up to your elbows. Hold for 5 to 7 seconds, then let go all at once. The warmth and heaviness in your hands is the release. Memorize that feeling.
- Shoulders and whole arms. Pull your shoulders up toward your ears and flex your biceps, like you're showing off your muscles. Hold, then let your shoulders drop. Do this one twice: almost everyone's shoulders are tighter than they think.
- Face. Squeeze your eyes shut, scrunch your forehead, and clench your teeth (moderately). Your whole face gathers toward the center. Release: forehead smooth, teeth apart, tongue resting loosely behind your upper teeth.
- Neck. Gently: press the back of your head into the chair or the floor if you're lying down. Hold for 5 seconds, release. No sudden moves: your neck hates those.
- Chest and diaphragm. Take a deep breath, hold it for 5 to 7 seconds, feel your rib cage expand, then exhale slowly and notice your ribs settle.
- Stomach. Tighten your abs like you're bracing for a light poke in the belly. Hold, release. Afterward, your belly should move softly with each breath.
- Back and lower back. Gently pull your shoulder blades together and arch your lower back slightly, hold, then release and let your back sink into the support. If your lower back hurts, skip the arch and stick to the shoulder blades.
- Legs. First tense your glutes and thighs, then your calves (pull your toes up toward you), and finish by curling your toes. Hold each group for 5 to 7 seconds and release on the exhale.
- Finish. Rest for a minute or two and slowly sweep your attention from your face down to your feet. Found a spot still holding on? Run one more cycle for that zone.
How often to practice: the standard recommendation is once or twice a day for 2 to 3 weeks. The effect builds up: your body starts catching tension earlier and dropping it faster. Breathing and stretching: what to add to the relaxation
Breathing with a longer exhale. The exhale is your nervous system's natural brake: it activates the parasympathetic branch, the one in charge of rest. The pattern is simple: in through your nose for 4, out through your mouth for 6 to 8. Five minutes of this noticeably lowers muscle tone. Diaphragmatic breathing. Rest a palm on your belly and breathe so that the belly is what rises; your chest stays nearly still. This directly works a tight diaphragm: 2 to 3 minutes, a few times a day.
Gentle stretching. A slow head tilt toward your shoulder held for 20 to 30 seconds, shoulder rolls backward, cat-cow for your back, an easy forward fold toward your feet. One rule: stretch to the point of light pull, never pain, and no bouncing. Pain makes the muscle clamp down harder.
Heat and movement. A warm shower aimed at your neck and shoulders relaxes them as well as any exercise. And a 20-to-30-minute walk at a moderate pace lets you spend the adrenaline stress mobilized.
Micro-habits that keep tension from building up
A one-off relaxation session clears what's already accumulated. To keep the knots from re-forming, you need short check-ins throughout the day. Each takes under a minute.
- A body scan on a timer. Set 3 or 4 reminders a day. When one fires, take 30 seconds: what's happening with your jaw, shoulders, stomach? Found tension? Exhale and consciously let the zone go.
- The jaw rule. Lips together, teeth apart, tongue resting on the roof of your mouth. Check yourself whenever you're concentrating: for most people, that's exactly when the teeth are clenched.
- A posture timer. Once an hour: stand up, drop your shoulders down and back, do three slow shoulder rolls, and shift your gaze from the screen to something far away for 20 to 30 seconds.
- Piggyback on your routine. Attach the check-in to things you already do: the kettle heating, an elevator ride, a red light: one-second scan.
- An evening ritual. Five minutes of short-form progressive muscle relaxation or stretching before bed. It unloads the day's buildup and makes falling asleep easier.
When muscle tension means seeing a doctor first
Stress is the most common reason healthy people feel stiff and knotted up, but writing every pain off as nerves is risky. Rule out medical causes first if you have even one of these signs:
- the pain is severe, gets worse day by day, or wakes you up at night;
- numbness, tingling, or weakness in an arm, a leg, or your fingers;
- sharp shooting pain on movement, dizziness, ringing in your ears;
- the tension appeared after an injury, a fall, or a car accident;
- the pain comes with a fever or unexplained weight loss.
With any of those, see your primary care doctor or a neurologist: you need to rule out a pinched nerve root, a herniated disc, and other conditions that relaxation won't fix.
